The sprain in ligaments leads to over extension of big toe, in which the body weight is pressed through ball of foot, due to repeated stress; sudden increase in pressed weight (playing on hard surface); wrong shoe selection for strenuous activity; injury and; existing diseased state leads to turf toe. It might cause big toe pain and Hallux rigidus (stiffness of big toe). 

Symptoms of Turf Toe

It includes pain, swelling and limited joint movement (Hallux limitus) in big toe due to repeated stress and in some cases, a forceful motion leads to an injured state within 24 hrs worsening state of movement.

Different states of gravity of Turf Toe

It could be mild sprain (I); severe sprain (II) and; toe dislocation (III). The first two stages are treatable with protection and rest while third stage require surgical interventions

Why this is more common in ballerina dancers or football players?

What is the prognosis and treatment of Turf Toe?

The condition could be worsened if left untreated, thus sports activities are curtailed immediately and elevation of foot, icing of area and custom othotics are primary remedies. Bracing the toe to limits its overextension is a complex bandaging pattern can be achieved by expert only. Use of metal insoles and anti-inflammatory medicines I s recommended. Treatment is long and sometimes recovery pattern is extremely slow, thus early stage intervention is essential to prevent aggravation. 

How it can lead to Hallux Rigidus?

The primary cause of Hallux Rigidus is not known, however it has been observed that injury to a turf toe with progressive arthritic state might leads to Hallux Rigidus. It initiates with stiffness (minor loss of range of motion)-(I) aggravated to greater loss of motion- (II) severe cartilage loss –(III) severe loss of range of motion and cartilage (IV) requiring joint replacement as last option in stage IV.  

What are the symptoms of Hallux Rigidus and how it can be diagnosed?

As the name applies, it is the rigidness of big toe while pushing it off during active walking which causes pain and swelling in joint region. A bump like bunion or callus might emerge on top of the foot and eventually inability to bend it up and down, consult you doctor immediately. It can easily be diagnosed by physical examination and X-rays. 

How Hallux Rigidus can be treated?

The primary cause is inflammation and pain, which needs to be reduced by anti-inflammatory and pain relievers. Contrasts baths using cold and hot water (alternatively, avoid extreme temperatures) can reduce both stiffness and pain. Avoid wearing high heel and wear a shoe with high toe box. A stiff soled shoe with rocker or roller bottom design with a steel shank in the sole to reduce the amount of bend required in big toe.  The conditions like bunion and calluses developed requires orthotic support for Toe Care while Toe Cap could be recommended in case infection is also accompanied. 

If such conservative care isn't able to support the surgical treatment intervention include spur incision; fusion of the great toe and joint replacement.
